Gateway fee auto-renews monthly at $0/mo. + taxes & fees for 12 months, after which price will go up no more than $5/mo. per year until standard rates reached. Limit 1 per household. Total Care Plus is $15/month for the first 12 months from the date of enrollment. After 12 months, the monthly price becomes $20 and will automatically renew each month until cancelled. Cancel any time by writing to administrator at P.O. Box 1818, Sterling, VA 20167, at 855-993-1680 or by logging into account at optimum.net. Cancellation within the first 30 days effective immediately and subject to a full refund. Cancellation thereafter effective immediately subject to pro-rated refund.
