Offer is limited to active Cablevision customers who subscribe to cable (OptimumTV), digital cable (iO Interactive Optimum), high-speed cable Internet access (Optimum Online or Business Class Optimum Online), and/or cable modem voice service (Optimum Voice). This offer is only available to the end user and original purchaser who's name appears on the Cablevision account. To be eligible for a rebate, you must purchase a qualifying product or service via the Optimum Store, register for your rebate by completing and submitting an official rebate form on www.checkmyrebate.com/optimum/register.asp within 45 days, and retain your product or service through the manufacturer-designated return deadline specified at the time of purchase. Cablevision and Optimum Store partners are not responsible for late, lost or misdirected mail or submissions. Limit one submission per qualifying purchase, per person, per address or household. Offer available in the U.S. only.

Rebate certificates will be sent in the form of Optimum Cash made payable to Cablevision and will only be redeemable against your Cablevision service invoice. No change will be given. Any remaining rebate balance will be applied to a subsequent Cablevision bill. Not redeemable for cash. Cablevision reserves the right to reverse the rebate (i.e., repost rebated charges) on any Cablevision subscriber account if the Optimum Cash or other rebate certificate applied against the account was issued for a service or product subsequently cancelled or returned. All certificates must be redeemed within 180 days of issuance or they will be forfeited. No mechanical reproductions will be accepted. Rebate certificates, which have been altered or tampered with, will not be accepted. Please allow 8-12 weeks following qualifying purchase to receive certificates.
